    I think the exercise is from Arnold Jacobs "Special Studies for the Tuba". You can actually download the entire thing. The picture you have is on page 13. It is probably not the same book you have a picture from, but perhaps this will help.

    Go to this page: http://www.hickeys.com/pages/tume2.htm - You can buy the Hal Leonard book Advanced Band Method - Tuba by Rusch and Jacobs. I think this has the Etude 6/4 on page 58.
